Important recommentations before your preoperative assement at the clinic

The IRIS Ophthalmology Clinic team will be happy to welcome you in a calm and safe environment for your preoperative assessment, which will last approximately two (2) hours. In order to optimize your visit and allow our professionals to respond to your needs, we invite you to read the following instructions carefully:

STEP 1: COMPLETE THE ONLINE MEDICAL QUESTIONNAIRE BEFORE YOUR CONSULTATION

In order to speed up the admission process and the opening of your file, we suggest that you complete your medical questionnaire online, before your virtual consultation or your preoperative assessment at the clinic. The information collected will allow our professionals to identify your vision needs and assess your candidacy for surgical vision correction.

The questionnaire is divided into sections. It should take you less than ten (10) minutes to answer all the questions.
Answers will be transmitted in a secure manner, ensuring confidentiality and protection of your information.
A copy of your questionnaire will be emailed to you once completed.

STEP 2: SEND US A SUMMARY OF YOUR OCULO-VISUAL FILE

With your consent, your vision professional may send us a summary of your optometric file, before your virtual consultation or your preoperative assessment at the clinic.

Contact your optometrist to ask him or her to fill out the Preoperative referral form, available online for professionals, in the “Referents Section”. Information can be sent to the IRIS Ophthalmology Clinic:

If you are unable to reach your optometrist, send us your most recent prescription for glasses or contact lenses. This information will allow our professionals to assess the surgical correction alternative that best suits your vision.

STEP 3: PLAN TO STOP WEARING YOUR CONTACT LENSES

In order to ensure the accuracy of the preoperative measurements, please plan to stop wearing your contact lenses completely before the preoperative assessment at the clinic:

Soft contact lenses, daily wear (removed every evening): at least 7 days off.
Soft lenses, extended wear (worn overnight): at least 14 days off.
Soft toric lenses (correcting astigmatism): at least 14 days off.
Semi-rigid gas permeable lenses: at least 1 month off for every 10 years of wearing.

If you are not able to function with your glasses during the suggested period, the clinic will offer you a partial assessment as a first step.

Ideally, plan to remove your contact lenses at least 24 to 48 hours before the partial assessment.
If you are planning surgery, you may need to stop wearing contact lenses for the recommended period, for the second full preoperative assessment.

STEP 4: CHOOSE A DESIGNATED COMPANION

Dilation drops may be used to fully assess the health of your eyes.

It is strongly recommended that you have a designated companion as you will not be able to drive immediately after the assessment.
The effect of the dilation lasts approximately 4 to 6 hours.
Dilation can cause blurred vision and increased sensitivity to light. Bring sunglasses to improve your comfort.

If you are unable to bring a companion or to dilate, the clinic will offer you a partial assessment as a first step.

If you are planning surgery, you may need to schedule a subsequent visit for dilation to complete the preoperative assessment.

STEP 5: BRING YOUR IMPORTANT DOCUMENTS

To facilitate the opening of your file, remember to bring your useful documents to your consultation:

Your medication and allergy list, if applicable.
Your Quebec health insurance card (RAMQ).
Choose a method of payment for the consultation fee.
The cost of a consultation will be communicated to you upon request.
The IRIS Ophthalmology Clinic will deduct the consultation fee from the surgical fee if the surgery is performed within 12 months of the assessment date.

STEP7: LEARN ABOUT PREVENTION MESURES IN THE CONTEXT OF COVID-19 PANDEMIC

Experience better vision with complete peace of mind. IRIS Ophthalmology Clinic has rolled out a series of pandemic-specific measures to protect your health and the health of our staff.

Questionnaire on your health status and symptoms:

Upon your arrival, a member of staff will ask you to validate the answers to your questionnaire on your health status and your symptoms.
With your consent, a member of staff will take your body temperature using a non-contact thermometer (forehead level).
If you have symptoms and/or risk factors and/or a temperature over 38C, we will invite you to postpone your visit to a later date and contact Public Health.

Wearing a compulsory medical or non-medical mask:

Please bring your mask. Make sure it covers your nose and mouth.
Replace your mask if it is wet, soiled, damaged, or if it has been worn for more than four (4) hours.
If necessary, the clinic can provide you with a disposable mask when you arrive.

Hand hygiene:

Upon arrival, disinfect your hands and frequently used items (smartphone, tablet, etc.).
Hand sanitizers will be available at strategic locations in the clinic.

Physical distance:

During your visit, keep a distance of two (2) meters from other users and staff, when the situation allows.
Some evaluations may require proximity between you and the professional. During these procedures:
- Limit the conversation and wait for the professional to walk away from a safe distance to ask your questions.
- Keep your hands on your knees or in your pockets.
In order to limit the number of people circulating inside the clinic:
- Respect the time of your appointment. If you arrive early, stay outside the clinic until your scheduled appointment time.
- Ask your designated compagnon to wait outside the clinic. If your situation requires the support of an attendant, please notify our staff before the visit.
